Max, i don't know many powerscape therapists and i hope that you can othercome these problems, its best to deal with them one at a time, although i have never been subject to self harm, i can understand your emotions at this stage.
You should consult whomever you can, maybe surround yourself with good friends.
and dreaming big can help, like my exotic dream (I want to become famous, given the opportunity to become famous i would take it, and im waiting for that opportunity)
This kinda keeps me going when im down, believing i will become famous in later life
